PLEASE SCHEDULE AN APPOINTMENT

Once the online process has been completed and submitted, the student’s information will electronically be sent to the school. Please print ONE copy and sign your online documents.

(If you do not have access to a printer, school will print out copies for you)

In order to complete the enrollment process, it is required to setup an appointment with your schools office manager.

PLEASE BRING THE REQUIRED DOCUMENTS TO YOUR APPOINTMENT

PROOF OF RESIDENCY (Please provide TWO of the following documents):

  • Property tax payment receipts
  • Rental property contract, lease, or payment receipts
  • Homeowners or renters insurance policy
  • Current tax return
  • Utility service contract (only gas, electricity, water, trash accepted), statements, or payment receipts
  • Pay stubs
  • Voter registration
  • Correspondence from government agency
  • Certified original closing document from escrow or rental/lease agreement 

If you are unable to provide two of the above listed proofs, you will be asked to fill out the Declaration of Residency Affidavit.

  • Declaration of Residency executed by the parent or legal guardian of the student
    • (An affidavit of shared residency will be required if residing with another family)

PROOF OF AGE (Please provide ONE of the following documents):

  • Birth Certificate
  • Statement by the local registrar or county recorder certifying the date of birth
  • Baptism Certificate
  • Passport
    (If none of the options above are obtainable, an affidavit of the parent, guardian, or custodian, or any other appropriate means of providing the child's age) 

OTHER REQUIRED DOCUMENTS

  • Parent/Guardian Photo ID
  • Immunization Records for all grades
    • 7th and 8th grade enrollments must provide proof of TDAP upon enrollment.
  • Current IEP or 504 Plan or GATE
  • Custody documents
  • Any medical forms provided by your child's doctor

REQUIRED: (Items can be helpful in placing your child in the appropriate class or programs)

  • Last Report Card (6th7th, 8th grade)
  • Last School Transcripts (7th and 8th grade ONLY)

Enrollment is not complete until all required paperwork has been submitted to the school office.
